Question: is it ok to write Ph.D. after John Gillespie, before et al? Or should degree and affiliation be together?

I conferred with Lynnette on this one. She says:

The intent was that just the name would be on the first title page. If you look at 2.4.2a you will see the same example with all the degrees and affiliations for the second title page.
