    I understand the table of contents in this book should be print-page numbered as i-iii. The next (blank) page would then be iv. Should the first page of text be iv-1?

    Each group of print page numbers ends on the last page in that group bearing text. Each group of print page numbers is listed separately on the title page. See the examples given in Section 2.3 and other places in Formats. The last blank page of a group is has not text and doesn't need to be accounted for because there is no text-bearing page in that group after it.
