I want to address your original statement about teacher's editions not being governed by the rules of Braille Formats. That section of the Code actually says that the Formats Code does not address the special problems presented in the transcription of teachers' editions of textbooks. It does not say that the rules do not apply. So, as far as you can, you should be using the guidelines of Braille Formats, even in a teachers' edition.
