Thank you for sending the print page. I'm not sure what you are asking here. You refer to double italics and how to close them. Are you asking how to close double bold and double bold italics? Secion 5.4 gives several examples of various combination of italics and boldface that show opening and closing. Perhaps it would help if you indicate which specific text on your print example page you are questioning and we can figure out how to do it.
