The rule says to leave one blank line between the Title/subtitle and the author so that is what I would do.
In regards to blank lines between the groupings, you may put the word "By" on a line by itself above the Transcriber's name but it does not say we can do that with the publisher or copyright. Leave your blank lines between groupings starting from the bottom of the page and working upward. If that means 3 blank lines between those groupings, that is what you do.
