What is the format for extraneous material in a Table of Contents for a Grade 1 book?

There are no page numbers referencing the material.

    Below is an example:

    Look for Assessment Opportunities
    (box line)

Student Book A and Student Book B

(bullet) Quick Check at the beginning of every ...
(bullet) Guided Practice after every example or two ...

    Workbook A and Workbook B

(bullet) Chapter Review/Test in every chapter to ...
    (bullet) Cumulative Reviews ...

    end of box line

Every other page in the contents has this type of text.

    Thank you.

In order to assess how to handle the extranous material I need to see what it looks like. I need to see the entire print page so I can see how the different types of material printed on it relate to each other, especially if it's a table of contents and some of the material is not part of the table of contents.

Please scan and post a print page or two.

    Thanks.
