In problem 6 of the attachment, there is a stem, answer choices, and problems. Does the format for this follow more a UEB setting including the blank lines such as Sample 10-3 in Formats 2016 or closer to the subentry Example (3) in the yellow instruction manual p. 98 (couldn’t locate the similar sample in provisional lessons yet). Or in another scenario, would the stem be in 1-5, bulleted answer choices in 7-9 as Nemeth displayed material, and subentries in 3-5 even though the answer choices are words, not math?

It appears that what you have in your item 6 is a stem followed by a displayed bulleted list of non-technical material and a series of subitems. So, the correct formatting is probably 1-5 for the stem, 7-9 with blank lines before and after for the bulleted list, and 3-5 for the subitems.
