In the 2011 guidelines 13.8.1 states "a blank line is inserted between the end of a stanza, or the end of a poem, and the note separation line." Sample 13.8 on page 13-22 does not show a blank line. If it is not the end of the poem/stanza, just end of print page, is the blank line omitted?

Yes, if the poem is continued on the next page the blank line is not used. The blank line would imply the end of the stanza or poem. One way to tell that this is not the end of the poem is the colon at the end of the last line.
