true/false with write-on lines

Now that "true/false" is to be retained, suppose it is not repeated for every exercise item, but only shown at the top with write-on blanks for each question?

I don't have an actual page at the moment. This is a hypothetical question relayed from someone else. The print she's seen, though, looks just like I typed it.

Without a print page to provide context and actual placement, exercise directions (if any) and possibly a look at how other exercises are printed, only a hypothetical answer can be offered. Formats Section 10.10 should be followed regarding the T/F and 10.5, particularly 10.5.2 should be utilized regarding the numbered write-on lines.
