    Do I need to include the "Further reproduction or distribution in other than a specialized format..." notice if I have only received permission to transcribe one copy of a book from a publisher? It seems that I would not include it since they have not given permission to reproduce it more than the one time. I am not an "authorized entity" so I am required to get permisson from the publisher. I will include a "With permission" notice on the title page. Is that correct?

    Also, I'm not up to speed on the new Formats yet, but will any of the changes impact the literary transcription of a novel?

    Yes, the new guidelines will affect Literary novels. The guidelines for the Title page and front matter as well as some other areas affect both novels and textbooks. NBA's October conference in Texas, will have a workshop on how the New Guidelines will affect the Literary Lessons and manuscripts. That information would probably give you the answers you need.

    As far as your other question, I can't find any guidelines that say it is OK to leave out the reproduction notice when a publishers permission has been given. It might a good idea to include them both just to be on the safe side.
